Thank you for your interest in the Apostolic Christian Home of Roanoke. We strive for excellence in long-term care!

To apply for the skilled nursing facility, download and fill out the entire Application for Admission and the Financial Information page and email to: richard @ achroanoke.org (without the spces), mail it to the Home, fax it to (309) 923-7919 or bring it in to our front office. Once your application is received, you will be contacted for more information. You will need to choose a doctor from the list who visits our facility.

To apply for admission to the Country View Apartments or a duplex, fill out the first two pages of the application and email to above email address. The financial information page is only necessary when applying to the skilled nursing facility.
